9/30/2023 EnchantaVaughn Approved Behavior: Upon arrival, client was in a quiet and isolated mood, as evidenced by his standoffish demeanor and uptight posture. Client continues to show disrespectful manners. Client continues to state that he doesn’t want any additional assistance or support with his behaviors or feelings. Behavior: Client expressed feelings of panic and distress throughout the day. Client stated that he locked himself inside his room so that he wouldn’t have to associate himself with anyone. Intervention: QMHP went to the home for the purpose of meeting with client for individual session. QMHP engaged in conversation with client about his weekend. QMHP asked client if he practiced his calming strategies over the weekend. QMHP processed and reiterated with client about the rules in the home. QMHP assisted the client with understanding the expectations that have been established in the home and community. QMHP went over social skills activities with client to help him understand the importance of following rules. QMHP informed client of replacement behaviors in efforts to decrease attention seeking behaviors the client often is involved in. Intervention: QMHP activity helped client state, demonstrate, or identify how to communicate his feelings. QMHP processed with client the thinking questions to each social skill activity. QMHP will help client develop healthy self-esteem by reducing level of anxiety and emotional reactions related to home and other independent functioning. QMHP encouraged client to trust hi mself with every decision that he makes on his own. QMHP assisted client to behave in more positive ways by addressing the conflict he was facing with his parents in the past. QMHP modeled appropriate ways for client to get positive attention from his family. Response: Client informed counselor of his behaviors and attitude and how he was working on doing better and helping his mom more around the house. Client understood the rules in the home. Client understood the expectations that were established in the home and in the community, and how to conduct himself in each setting. Client participated and cooperated well with the social skills activities. Client was able to answer the thinking questions with assistance from QMHP. Client communicated that he feels that he will be more productive in the home, due to consistent support in the home and seeing QMHP 4x a week. Response: Client followed all directions during the session. Client incorporated numerous strategies that he learned from his previous sessions, such as thinking before he reacts and not interacting with negative emotions in the community and at home. Client needed help understanding instructions on numerous occasions but after understanding the concept he was able to participate very well. Client verbalized that he’s able to focus better when it is quieter and more peaceful. Client took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time instead of being all over the place and getting easily distracted. Client did not display any outbursts during the role play exercise.
